My guess would be that you have to go to the players' club to exchange the LVA coupon for a coupon you can use at the tables. They probably require you to join the players' club, and they make a notation that you can get subsequent coupons in succeeding quarters.
The first time you play, detach the MRB voucher at the table. Subsequently -- it can be played once per month -- you will redeem the MRO at the Club Services booth for validation that you will take to the table.
