Have eaten there two or three times a year for the last 5 years or so.

No lunch .... The Flame is open for dinner only (opens at 4pm).

Once inside, you can't wear a hat. If wearing one, you will be asked to take it off.

Dress code is casual but I don't think jeans and a t-shirt would be appropriate. I've never seen anyone in there dressed that way but, then again, I've never seen anyone dressed that way turned away either.
It's not open for lunch.

I've only been coming to Las Vegas since 2004, so I've missed out on most of the old-school gourmet rooms. When I get a chance to go to one, like this or Hugo's Cellar, I like to dress up just a little bit. I don't go as far as a coat and tie, in fact, I never take either of those with me, but I always take a nice pair of khakis and a nice knit shirt and I'll wear those when I go to a show or an old-school room.

I always find myself wishing I had gone to LV "back in the day". People love to criticize the way the town has changed, but I think that the increasingly vulgar way people dress and act is more a reflection of our society than just of Las Vegas, so for me, dressing a little nicer is my way of paying homage to a more genteel time - one that will probably not return. It's my nod to my old-fartdom.

As I descend from my soapbox, I'll say that I've seen people in the Flame in jeans and in shorts and that they don't seem out of place.

I plan on visiting The Flame in a couple of weeks and I was perusing the menu. Do the entrees come with any sides or do you have to order all of the sides separately?
Separately & two people can easily split one side.

The mac and cheese is great, too. And as prabon said, one side is definitely enough for two people. Of course, they're only about five bucks apiece, so you could order mac and cheese and, say, asparagus, which was also very good when I had it, and not worry about being able to finish it all.
