A man is a human being who was born with the requisite organs, genitalia, hormonal array, glands, and reproductive equipment. A man can also call himself a woman and alter as much of that original equipment as he wants, as well as his dress, appearance, and mannerisms. That said, in modern society, and to a great extent, all past societies as well, gender identity has been as much about appearance and behavior as anatomy --and usually, one cannot see the anatomical features that would make it unequivocal. There have always been men who looked like women and vice versa--on purpose or not.
Therefore, if someone wishes to alter their appearance and behavior, I feel that they should be treated as a member of the gender they wish to appear as.
