Well, using that metric, we shouldn't give the vaccine to people whose underlying conditions are a result of their poor lifestyle choices---like someone who weighs 350 pounds and smokes. (This may seem social Darwinist, but a similar approach has been discussed in providing universal health care---prioritize access for people who pay more attention to their own health.)
I'm reasonably sure that if a healthcare worker spends all day in an office and not treating or contacting patients, that person would refuse to be prioritized. That wouldn't even be an issue if healthcare workers were classified by their degree and frequency of their exposure to potentially infectious patients.
In Washington state, there was a group of 300 young Republicans who held some kind of idiot gathering. They were originally going to have Sarah Palin speak but she chickened out. They posted videos flaunting their masklessness and taunting Governor Inslee. Also in WA, in Chehalis (a small town halfway between Seattle and Portland), a restaurant has stayed wide open in direct defiance of the governor's orders---they even posted that on social media---and now, idiot Republicans are traveling up to two hours just to eat there. The place is jammed.
Should any of those people get a vaccine? Ever?
