He doesn't even know what a lie IS, which doesn't stop him from calling all and sundry liars. That's what got him thrown out of the Four Queens.

Yeah, though it's happened before, and it was only temporary, like a drunk getting heaved out onto Fremont Street. David's too valuable to them for him to bar him permanently.

The players at Four Queens table games are rowdy and real screamers, which is why David really has to push the envelope to get tossed out. It's so stupid for him to call the dealers LIARS LIARS LIARS when he busts--anyone can see that a 10, a 2, and a Q totals 22--that initially, they're just amused--oh, just another senile old wacko--but their patience wears thin after a while. Those dealers must have incredibly thick skins and cast-iron dispositions. With all the pigarette smoking, the drunks, etc. etc , they really do earn their money.

Why was he asked to leave Four Queens? Do you know the actual story?

 The whole lie about me being "kicked out' of the 4 Queens is a fabrication. Never happened. Lewis is pulling your chain(s).

I have three long-time friends who work there--two in the pit and one in security. They told me of "an old guy from Texas" who is a frequent visitor and has been thrown out multiple times for abusive behavior toward both the dealers and his fellow players. 

As Miller always announces his trips, I've asked them if that person was there during those times. They've always coincided.

That is only circumstantial. However, Miller has threatened to find me in Vegas and "teach me a lesson"--multiple times. As I am also a frequent 4Q visitor, I didn't want to take even the negligible risk that he was some crazy who would actually try to find and attack me. So I asked my friends to keep an eye out for "the guy from Texas" and for advice re what I should do if someone threatened me in the casino. They told me once, while I was there, that he was there as well. They also said that due to privacy issues, they couldn't give me his name or a physical description. So I simply stayed away from the pit, and ultimately nothing happened.

You don't know the backstory, which was that I had an LVA blog, and I said something mildly disparaging about Trump (it was 2015), and Miller's response was to flood my Comments section with thousands of gibberish posts--daily. It consumed an hour+ of my time every day to take them down. I finally got tired of it and quit. Miller claims that Anthony made me take down the blog. I counter with the offer that if he can get Anthony to confirm that allegation, I'll leave LVA altogether and never return. At that point, Miller becomes strangely silent.

So I don't know if the nasty old guy from Texas is him. But I have other confirming info that I can't reveal, because I don't want my friends to get in trouble. 

It's certainly at least theoretically possible that Miller is unfailingly polite when he visits the 4Q, and that old nasty guy is someone else. But boy, is he angry when after three or four days in Vegas, he comes to the Sink and starts dozens of threads and screams at everybody.
