Where have they gone?

You have to pardon Tom. He is an advocate for euthanasia as a solution to America's healthcare problem, but it is not as bad as it sounds. Most Republicans want to kill the sick slowly by keeping them from getting treatment. Tom wants to put them out of their misery so they don't have to go through all that pain and suffering.  

Once again mark makes things up.

Well, Mark, you know that the problem with universal healthcare is that it allows the inferior classes to live longer than they should. Also, by the sacred tenets of social Darwinism, you can't earn the money for that expensive, life-saving operation, you don't deserve to have it and should die. For Republicans, there's a bonus in denying everyone healthcare--doing so will extend the reign of the rich old white guy a little longer.

We're giving fritzie too much credit, and too much attention. That's what he wants. The onset of puberty has been an ongoing emotional struggle for him. Especially since it appears to have taken 40+ years for him.
