Possibly scarring from repeated injections? If he's being treated for CHF, he would be receiving diuretics, intravenously or orally.
Normally, such treatment wouldn't leave a scar that large, but Trump probably swats at and bites every nurse who gets near him. He doubtless fires each one after they stick him with a needle.
It could also be that they periodically substitute a wax dummy from Graumann's Museum when he wants to stay home and watch Fox Nooze instead of doing his job.

But, yeah he is getting some powerful IV diuretics and the fact that it isn't reducing the fluid around his ankles means he doesn't have much gas left in the tank.

That's sad. Not because he's going to die--that will eventually be a national holiday--but because a heart attack would take him out pretty quickly and relatively painlessly. He deserves to die a million deaths, but since he only gets one, it should at least be prolonged and excruciating.
