We were in Vegas last week. My card expired in October 2010. I received new Sapphire card with an expiration date of October 2011. This indicates a year to year renewal on the date of the anniversary when obtaining your second level. My brother was Ruby (no expiration date) and had enough for Sapphire. The booth folks gave him a new Sapphire card with a one year expiration date.

I also discovered you can also receive buffet comps without using your Bonus points. This past Friday, the Seafood buffet line extended to the casino floor at MSS. I walked over to the slot booth and inquired about a line pass and was given a comp buffet for two. We then used it at the VIP line.
MSS is pretty easy for a buffet comp, they're generally looking for about $1500 coin-in and don't deduct your points; any players club worker can give you this.

And as you've noticed, the tier points (I think they're called card score now) expire in 365 days, on a rolling schedule (i.e., the points that you earned on Feb. 10, 2010 are gone on Feb. 10, 2011); you need to maintain over 30,000 to keep Sapphire status (it says within 6 months in the brochure, but I think it may be 12 months for out-of-towners).
