Kevin Smith had just left the game because of injury, so they were not comfortable running the ball, although I do not know whether he is the short yardage guy anyway. (They may not even know who that guy would be because the opportunity occurs so infrequently.)
The fourth down throw was out of the end zone because there was a DB blitz, and he had no time to do anything. It was not his intention to throw the ball away; his OL did not give him a chance to see anything.
The FG was from the 5, I believe, and it was the right play because it put the team down two scores. It was definitely a mistake to kick it deep, although they did stop them before throwing a pick 6. If the hands team was out there for GB, then perhaps kicking it deep wasn't the worst idea.
After the pick six they should have put Culpepper in. Stafford's proved his gutsiness to his teammates, so the respect is there. Hopefully he will still be alive to take advantage of it. Aaron Rodgers has been sacked so many times this year that it's going to make him the next Marc Bulger. This could happen to Stafford if they keep leaving him out there to get hit.
