He may want to consider seeking mental health help, maybe a licensed psychotherapist, psychologist, or psychiatrist. By refusing treatment in essence he is choosing passive suicide. He may not see any hope where a professional maybe can help him explore alternatives. If his local community has a public mental health team inquire if they could send out mental health professionals to see if referrals can be provided. The mental health team could even put him on an involuntary 72 hold if deemed necessary. Another option might be to contact his local Adult Protective Services (APS) and see if they can open up a case for him. You could do this anonymously if you so choose. My recommendation would be to never give up on a loved one. Always keep hope alive. When all is said and done, you don't want to regret contemplating if more could have been done.
