Q:

My wife needs assistance using the restroom. Are there any downtown casinos that have family/assistance restrooms like many rest areas on the interstates are starting to make available to travelers?

A:

FindFamilyRestrooms.com, unfortunately, doesn't list any downtown. They do list a number on the Strip: at Bellagio, Mirage, Wynn and Encore, Aria, Cosmopolitan, and Excalibur, along with the Forum Shops and Fashion Show. They also cite two at Aliante, one at Red Rock, one at the Meadows Mall, and five at McCarran. They include the locations of these restrooms at each venue. 

However, doing a little more sleuthing, we found that there's a gender-neutral restroom at the Golden Nugget, in an upgraded area on the ground floor near the escalator that went up to Gold Diggers nightclub before it closed.

Another tip we can relate is that a high-limit area might have a dedicated and oversized restroom for its VIPs, whom they don't want to have to wander out onto the casino floor. 

If anyone has more info about unisex restrooms on Fremont Street, we'd appreciate the comments.

    There is one at Arizona Charlies Decatur, by the gift shop.

    Where ever you are, ask an attendant to block a restroom for the short time needed for this if they have no unisex.  They should be willing to make it happen for her.  Even speak to somebody about it ahead of time, how they can facilitate this need and not be stuck in an uncomfortable situation.
    
    The hotel where you stay, request a room on a low floor, close to the elevators, to expedite the quickest possible up trip to your room when needed.  Call hotels you are considering, ask about modified rooms, how many, can they reserve one for you for day of arrival.

    It's downtown, so...
    These are all older casinos and wouldn't have had upgraded restrooms. Retrofitting any existing restrooms might be impossible or at least, prohibitively expensive.
    
    I don't agree that any casino employee should "block" a restroom--even for a "short time." Five minutes can seem like an eternity when you need to pee, and while I believe in accommodating the needs of the disabled whenever possible, I do not believe that the needs of the "abled" are any less important.
    
    The simplest solution may be to get a room at one of the downtown hotels, and there's your private bathroom. You could probably venture across or down the street without being too far away when duty calls.

    Regarding Family Care restrooms at the downtown casinos, there is one at the downtown Grand.
    
    There's also one at the Gold Coast at the entrance to their upscale Steakhouse but you will need to check with security to gain entry.
    
    There's also one in the Tropicana in back by the food court.
