Exactly what information is transmitted from a VP machine to a casino host? Is it just wins/loss and time played on the machine. If you play several machines in a session are each compiled separately or are they combined?

The short answer is all of the above. The host, and other rewards representatives, can see as much or as little detail as they want. Every machine you play on generates its own separate rating that shows machine number, denomination, time started, money in, time played, average bet, win/loss -- in short, everything.
There's also an option with which hosts and reps can divide play into "trips". If you play over a three-day period, which is normal for a lot of players, it will group all your stats for that trip into an overall view of win/loss, average bet, game types, and again everything for that trip. That way hosts can see your play throughout various trips.
Of course, there's also lifetime history, which does the same thing over the entire period encompassed by your players club account. If the casino has sister properties, or the rewards club involves other casinos, the play for those is usually accessible through another screen.
For the record, this is also how ratings are established for table games. Each table generates its own rating. Play can be viewed in session, trip, and lifetime format, with access to other properties if there are any.
