First off a couple of qualifications. 1. Obama has asked for an En Banc review which means all of the judges currently sitting on that circuit court will review this decision of the 3 judge panel. If the majority agree with the decision it will be final binding law on that Circuit. 2. Four hours after this ruling the 4th Circuit Court of Appeals issued a ruling that contradicted with this ruling.
What this all means is that assuming the En Banc DC Circuit approves their three panel decision the Circuits are in conflict creating an issue that the Supreme Court will decide. Given the recent privately held for profit corporations have religious freedom decision coming out of there I don't think it is too hard to figure out that they will likely affirm the ruling coming out of the DC Circuit.
As a practical matter what it means is there will be no more subsidies for people buying health insurance in the States that have opted to go with the Federal Exchange system (36). It will also mean people that have enrolled in Obamacare and have received premium subsidies will have to pay them back unless there is some sort of congressional fix (unlikely). So several million people could be getting $10k-25k bills from the IRS at the end of the year depending on the amounts of their premium subsidy.
